I picked this up and noticed the bottom pusher seems a bit more stuck out. Does this look like it to you?
What would you do?

You should have it tightened by a watchmaker. The cap is coming unscrewed.

Yep - movement needs to come out and then the screw can be tightened.

Years ago I removed a movement from a Mark 2 that had a broken crystal and glass floating around. It made me a bit nervous but I was successful. I actually put it back into another Mark 2 case for the time being. I am entertaining the idea of doing this myself but first I'll ask a local watchmaker whom I sort of trust. I don't know him well enough to really know. But I don't want to loose this!

I’d say that this something you can do yourself if you have some tools and you’re comfortable pulling the movement. It’s a fairly simple repair and something I’ve done on a couple of occasions.

If you don’t want to do it, I’d recommend putting the watch into a ziplock. The cap doesn’t look like it’s about to fall off, but why take chances.

I was doing some light reading on pusher replacement, etc..., and I thing this tool is needed: Bergeon 5901-P27 to tighten the pusher button. I don't really need it at this point I don't think. My question is, "why do people replace the pushers if they still have originals?"
I see people making comments like, "all original except the pushers" and it seems to hurt the value. Do they simply wear out? It seems they probably do over time but is there a gasket inside and the spring that can simply be replaced, keeping the original?

You only require that if you are replacing the entire pusher assembly. To tighten the button, you just need a screwdriver - 160 size.

If you want to wear it, best to just get it serviced and this will be fixed as well as likely other things that need attention.

I ended up just doing it myself. Had sufficient tools and barely enough nerve. I will eventually be sending it in at some point for a service. I found some smudge marks on the inside of the middle case. Movement is beautiful. No marks before or after! The case gasket was ok but not super great.
